For the past five years, i have been receiving treatment at another office which left me with bad experience, until a year ago when a friend told me about Smile by Design. I am writing this review to tell you how satisfied I’m coming to Smiles by Design. I have been a patient for two years now and needed full smile make over and wanted a doctor and office that cares about patient needs as opposed to their money. Not only was this office reasonable with pricing but the type of care I received, will never be reciprocated anywhere else. Doctor Abazie and his staff were professional and caring. First I was fully educated about my needs and they took the time to listen and answer all the questions I have. I was very impressed with every effort they made in order to satisfy my needs. I received eight veneers to correct my stained and cracked front teeth. Also I received three implants in the back of my mouth to replace missing teeth. My confident is now out of the roof and I can smile constantly. I have joy because of what Dr. Abazie and staff did for me. I thank my friends who are patients here for referring me. They are great people and treat their patients like a family. I highly recommend you make this office your dental home.

I went in to get my cleaning and check on my teeth after a few years of procrastination and I enjoyed the experience. The receptionist was quite welcoming and the hygienist was very pleasant as she educated me on what she was doing. I appreciated that, as well as her being courteous. I got to learn more about the health of my teeth. Dr. Abazie and the assistants were very attentive to my concerns. They listened to me and that made me feel very comfortable. How they educated me along with the use of modern technology was quite impressive. I can be hard to please at times and have visited many dental offices and I will say to you that this place is on top of my list of dental offices I have visited. I highly recommend them to anyone looking for a dental home. Also my treatment went very well and I look forward to continuing my treatment here. I’m glad I found this place. I Had to make an appointment for my husband too 😊.

When can I start using my membership?

Your membership is effective immediately!
How do I schedule my preventive care?

Simply call our office and setup an appointment for yourself or your family members.
How can I use my Member Exclusive discount for restorative, elective or cosmetic treatment?

Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is “current” and you will be eligible for instant savings off our regular fees.
How do I know if I need the Perio Membership?

If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.
What happens if I miss an appointment?

It will be your responsibility to reschedule. Please see our office policy regarding no-shows and last minute cancellations.
Can I add members to my account?

Absolutely! Your entire family can join.
Can I transfer my membership to another dentist?

Your club membership is solely with our office and as such is non-transferrable. If you move to another area, you may cancel your membership at any time.
How can I check on the status of my account?

Login to your Member Portal and look under Account Members. You’ll see the status of each member above his/her name.
Can I change or update my billing information?

Yes! Simply login to your Member Portal and click “Edit Your Payment Method”.
How do I cancel my membership?

Login to your Member Portal and click the “Remove” button for each member you’d like to cancel.
If I cancel, can I get a refund on my membership?

No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.
How much do I pay to sign up for my club membership?

You’ll pay only the Lifetime Activation fee to join. This fee includes your first month’s membership dues.
Can I prepay my membership fees?

Memberships are currently month-to-month only.
What information is required for me to sign up for my club membership?

If a patient is selected for free smile makeover through freesmile.life campaign, the patient is responsible for maintenance charges, and charges of procedures done as a result of failure of prosthesis, reinfection, treatment of complication that resulted from the free care as well as cleaning and lab charges. Patients must be an active member of the Smile Membership Club to qualify for the in-house low-budget dental plan and must be non-insured.
